I've never bought one myself, but there are a few sites that offer this service:

http://personalized4u.com/ seems to have the most variety available. I've never ordered from them, though.

http://www.mystorybook.com/ offers personalized books plus weaves parts of the child's life into the book -- takes a fair amount of work on your part and the con is that I've never ordered from them myself, so can't vouch for them and it doesn't offer traditional titles. The pro is that the personalization exists throughout the entire book.

Personally Yours Books have numerous popular titles (Sesame street, Looney Tunes, and Marvel comics, for example) The con is that I've never ordered from them myself, so can't vouch for them, and the personalization is only on the FIRST page of the book. Amazon.com has a few "personalized" books, the con is that they don't come that way -- you receive the book, fill out an info sheet, mail it away and get stickers back that you put throughout the book. The pro is that I've successfully ordered from them many times in the past (though never these titles).
